Getting Ready for the 20-21 School Year
Hello all! As we know the 2020-2021 School year brings lots of uncertainties of what’s to come but now is the perfect time to get all your paperwork together for sports. All student-athletes must turn in a completed Physical as well as Concussion Education to the Athletic Trainer.
The VHSL Physical must be on the 2017 VHSL Physical Form. The student-athlete and parent need to complete pages 1,2, and 4. Please fill out the form in entirety to ensure that it does not become a problem physical. Page 3 must be completed by the Dr.’s office. You can find a blank VHSL Physical form under the Team Files link.
CareNow in Chantilly is currently offering Physicals for only $20 until August 31st. You can visit https://www.carenow.com/patient-services/medical-tests/sports-physical.dot for more information. The Concussion Education for the 2020-2021 School Year has been updated since last year however it will remain a paper version. You can find the Concussion Education presentation in Team Files under the Sports Medicine tab (which can be found in year round sports). You will pen the attachment and review the PowerPoint. Once you finish the presentation the student-athlete and parent will sign the last page and submit it.
The last form needed will be an Emergency Care Card. This can also be found under Team Files in the Sports Medicine tab. The ECC will be submitted to the coach of the sport the student-athlete is intending to play.
