Physicals required by September 8th!!
As workouts begin on September 8th students will be required to have a VHSL physical in order to participate!
The VHSL Physical must be on the 2017 VHSL Physical Form. The student-athlete and parent need to complete pages 1,2, and 4. Please fill out the form in entirety to ensure that it does not become a problem physical. Page 3 must be completed by the Dr.’s office.
CareNow in Chantilly is currently offering Physicals for only $20 until August 31st. You can visit https://www.carenow.com/patient-services/medical-tests/sports-physical.dot for more information.
The concussion education for the 2020-2021 School Year has been updated since last year however it will remain a paper version. You can find the concussion education presentation in Team Files under the Sports Medicine tab (which can be found in year round sports). You will just open the attachment and review the PowerPoint. Once you finish the presentation the student-athlete and parent will sign the last page and submit it. This will be needed by the time their season begins.
The last form needed will be an Emergency Care Card. This can also be found under Team Files in the Sports Medicine tab. The ECC will be submitted to the coach of the sport the student-athlete is intending to play.
Ms. Bishop will also be setting up a physical drop off next week on Wednesday from 4-7pm and on Thursday from 11am-2pm outside of door 10. If you are unavailable to make the drop off you can email your VHSL Physical, as well as Concussion Education to firstname.lastname@example.org.
Please do not hesitate to email or call 571-363-7835 if you have any questions at all about paperwork.
Thanks and Stay safe!
